Pre-eclampsia Educational Tool Impact on Knowledge, Anxiety, and Satisfaction in Pregnant Women: A Randomized Trial

      Abstract

      Objective

      This study aimed to evaluate the impact of educational tools concerning pre-eclampsia on knowledge, anxiety, and women's satisfaction.

      Methods

      The investigators conducted a randomized controlled trial from March to July 2014 at the Centre Hospitalier Universitaire de Sherbrooke (Sherbrooke, QC) by comparing ambulatory pregnant women (20–32 weeks of gestation) who were receiving educational tools on pre-eclampsia with control patients who received routine care. Tools consisted of an informative pamphlet, a video, and a pictographic magnet, all validated by a multidisciplinary team. The primary outcome was global knowledge (number of correct answers on 35 items) about the disease after 1 month, as assessed by questionnaire. Secondary outcomes included anxiety regarding pre-eclampsia and satisfaction concerning the different tools (a 1–6 Likert scale was used).

      Results

      Among 362 pregnant women approached for the research, 269 were randomized. After 1 month, 247 questionnaires (92%) were filled and analyzed: 122 from the control group and 125 from the intervention group. Baseline characteristics were similar between the groups. Patients who received the tools scored significantly higher on global knowledge (70.1% ± 19.2% compared with 51.1% ± 23.4%; mean difference of 19.0%; P < 0.001). Anxiety scores regarding pre-eclampsia were similar between the groups, with a mean of 2.40 out of 6 for the control group and 2.53 out of 6 for the intervention group (equivalence test, P < 0.001). High score levels of satisfaction for the pamphlet and video were found: 5.1 out of 6 and 5.2 out of 6, respectively.

      Conclusion

      These well-received educational tools for pregnant women increased their knowledge about pre-eclampsia without increasing their anxiety about it. Women reported the highest satisfaction for the tools.

      Cette étude visait à évaluer l'incidence de ressources éducatives concernant la prééclampsie sur les connaissances et les degrés d'anxiété et de satisfaction des femmes.

      Méthodologie

      Les chercheurs ont mené un ECR de mars à juillet 2014 au Centre hospitalier universitaire de Sherbrooke (Québec), durant lequel des femmes enceintes ambulatoires (20 à 32 semaines de gestation) ayant reçu des ressources éducatives sur la prééclampsie ont été comparées avec des femmes enceintes ayant reçu les soins usuels. Les ressources, soit un document instructif, une vidéo et un aimant pictographique, avaient toutes été validées par une équipe multidisciplinaire. L'issue principale de cette étude était les connaissances générales sur la maladie (nombre de bonnes réponses sur 35 questions) après un mois, évaluées à l'aide du questionnaire. Les issues secondaires étaient l'anxiété à propos de la prééclampsie et la satisfaction à l'égard des différentes ressources (une échelle de Likert de 1 à 6 a été utilisée).

      Résultats

      Parmi les 362 femmes enceintes ayant été approchées pour participer à l'essai, 269 ont été réparties aléatoirement. Après 1 mois, 247 questionnaires (92 %) ont été remplis et analysés : 122 provenaient du groupe témoin, et 125, du groupe d'intervention. Les patientes qui avaient reçu les ressources ont obtenu un résultat significativement plus élevé sur le plan des connaissances générales (70,1 % ± 19,2 %, comparativement à 51,1 % ± 23,4 %; différence moyenne de 19,0 %; P < 0,001). Le degré d'anxiété à l'égard de la prééclampsie était semblable dans les deux groupes : la moyenne du groupe témoin était de 2,40 sur 6, et celle du groupe d'intervention, de 2,53 sur 6 (test d'équivalence, P < 0,001). Les degrés de satisfaction associés au document et à la vidéo étaient élevés et se chiffraient respectivement à 5,1 et à 5,2 sur 6.

      Conclusion

      Les ressources éducatives ont été bien accueillies et ont permis d'améliorer les connaissances des femmes enceintes sur la prééclampsie sans augmenter leur degré d'anxiété. Les femmes se sont montrées très satisfaites à l'égard des ressources.
